The majority of support is administered in the home or community and are available 24 hours a day, 7 days a week.  All mental health and substance abuse services are provided by the team with frequent contacts at least weekly but up to daily as needed.  Services include:

  • Intense medication organization and supervision providing security for medications 
  • Activities of daily living
  • Work related services
  • Supportive counseling
  • Social skill teaching
  • Peer counseling and support
  • Psycho-education with family

A typical PACT service recipient has:

  • Schizophrenia-spectrum disorder, bipolar disorder, or major depressive disorder with psychotic features; and
  • Significant functional impairments and (daily living, maintaining employment, homemaker roles, safe living situations)
  • One or more of the following:
  • Comorbid substance abuse; and/or
  • History of frequent or long-term hospitalizations; and/or
  • History of frequent arrests/incarcerations or homelessness episodes
  • Have not (or likely would not) successfully receive services from less intensive community-based treatment programs
