Case Management Services

Comprehensive case management links, monitors, advocates and coordinates among multiple services and resources to assist children who are at risk and their families with connection to needed supports.

Case Management Services Are For

Specific Services

  • Case managers assess individual strengths and needs. They help the child/adolescent and family to access resources in the community that address basic needs:
    • Educational
    • Financial
    • Medical
    • Psychiatric
    • Social
    • Vocational
    • Other need supports
  • Case managers coordinate services with other providers and agencies, provide advice and assistance, and monitor service delivery.
  • Individuals may receive services in their home, community, clinics, and other locations in the community.

Referrals

Children who are assessed to possibly benefit from Case Management by a Comprehensive Needs assessment, are referred to the program by the licensed Clinician who completed the Diagnostic Evaluation.
